Health Information Clerk

Job Number:    HAFP-HIM-410-01

Number of Hours:  Irregular Part-Time

Location:  Hancock, MI

Description:  Maintains an efficient and secure patient health information system through duties related to the collection, assembly, analysis, completion, filing, storage, retrieval and delivery of patient medical records.  Ensures the appropriate access to and release of confidential patient information through adherence to hospital/clinic policy and regulatory requirements.

Required Education, Training, Certification/Licensure and Experience:

  • High school graduate or equivalent
  • 6 – 12 months experience in an office setting
  • Medical record experience or other related clerical experience, preferred
  • Ability to access and input data into computer programs, preferred
  • Operate office equipment: computer terminal and printer, telephone, copier, facsimile machine, preferred
  • Able to sequence both simple and complex tasks in the correct order, preferred
  • Communicate clearly with other employees, medical staff and the general public regarding health information needs and concerns





Upper Great Lakes Family Health Center (UGL) is a Federally Qualified Health Center and is a Federal Tort Claims Act (FTCA) Deemed Facility.
UGL receives funding from The U.S. Department of Health and Human Services and has Federal Public Health Service deemed status with respect to certain health or health-related claims, including medical malpractice claims for itself and its covered individuals.
