New Location

Calumet Family Health Center will offer medical, dental and behavioral health services at their new location starting on March 27, 2017. The new facility will improve and expand access to health care in the Calumet area. 

Upper Great Lakes Family Health Center provides service regardless of the patients’ ability to pay. Most private insurance plans, Medicaid and self-pay patients accepted. Discounted rates available to those who qualify.

Upper Great Lakes Family Health Center (UGL) is a Federally Qualified Health Center and is a Federal Tort Claims Act (FTCA) Deemed Facility.
UGL receives funding from The U.S. Department of Health and Human Services and has Federal Public Health Service deemed status with respect to certain health or health-related claims, including medical malpractice claims for itself and its covered individuals.
