  • View Visit Summaries
  • View Upcoming Appointments
  • View Lab Results
  • View Diagnostic Imaging Results
  • Request Appointments
  • Request Prescription Refills
  • Request Referrals
  • Receive Reminders
  • Send and Receive Secure Messages to Our Clinic Team

You can even contact us if you have a question for your provider instead of calling our office! The best part, it's FREE! All of these features are free for Upper Great Lakes Family Health Center patients.

 

Note: The Patient Portal is not monitored 24/7 and should not be used in emergent situations or when a time-sensitive issue needs to be addressed. 

For these needs, please call your local UGLFHC office directly, or in the event of an emergency, dial 9-1-1.

How to Sign Up for the Patient Portal

New Patients: Enroll in the Patient Portal by visiting or calling any UGL Family Health Center location for assistance. 

Existing Patients: Enroll in the Patient Portal by visiting any UGL Family Health Center location or by clicking here.

After following the link above, patients will arrive on the Enrollment screen. Patients will need to:

  • Fill in the information on the right side of the screen (name, date of birth, ReCaptcha, and registered phone number)
  • Click "Submit"
  • Enter the verification code sent to your cell phone
  • Click "Verify"
  • Enter your personal email address
  • Click "Submit"
  • Click "Continue"
  • Create and submit a new password
  • Review and agree to the Terms & Conditions
  • Full access to the Patient Portal should now be available
