Upper Great Lakes Family Health Center respects your right to the protection and privacy of your personal health information. We make every attempt to protect your information so that it’s not heard, read or otherwise shared for any reason. A signed authorization form from the patient or legal guardian is required to disclose this information if not a Continuity of Care request.

The healthcare record is the property of Upper Great Lakes Family Health Center and is maintained for the use of any clinic and staff at the patient’s benefit. Please note that clinic staff are unable to release medical records.

You may request a copy of your medical records at any time. An easy, fast way to access your protected health information (PHI) and medical records is through your Patient Portal. Login – anytime, anywhere – to securely view your records or manage who sees your PHI. You may also request a copy of your medical records using the process below.

  • To fill out a release of information form, go to Patient Record Request | Verisma and click the blue button “Click Here For Record Request Form”.
  • To check on the status of your request, send an email to Status@verisma.com or call Verisma at (866) 305-4435. You should receive a response from a representative shortly.
