IMPROVING HEALTH & WELL-BEING

IN-SCHOOL WELLNESS SERVICE

The Houghton Elementary School Wellness Program is a collaboration of Upper Great Lakes Family Health and Houghton-Portage Township School District supported through funding from the Michigan Department of Health and Human Services (MDHHS). With parental/guardian consent, our Registered School Wellness Nurse and Licensed Behavioral Health Provider can provider healthcare conveniently within the Houghton Elementary School. This allows for ease of access, fewer disruptions of regular routines and less loss of class time. Our goal is to help improve the health and well-being of all students. Healthy students are better equipped for school success.

WELLNESS SERVICES PROVIDED*

  • Screening/nursing assessments
  • First aid for minor injuries
  • Dispensing over the counter (OTC) medications under medical director standing orders
  • Immunization assessment (review of record)
  • Chronic care intervention
  • Blood pressure monitoring
  • Case finding and case management
  • Point of care lab testing (upon request) and/or referral to other needed primary care and specialty medical services.
  • Behavioral health services upon referral

Monday: 8:00am-4:00pm

Tuesday-Friday: 8:00am-3:30pm

Adolescent Only Hours every Monday from 3:00-4:00pm

Walk-in hours every day from 8:00-9:00am

A provider is on-call after hours, on weekends and during holidays. For urgent after hours needs for family practice call (906)372-3025. For after hours needs for OB/GYN or Pediatrics please call (906)483-1000.
