IN-SCHOOL WELLNESS SERVICE
The School Based Health Center is located within the Lake-Linden Hubbell School (LLHS) and is a collaboration of LLHS and Upper Great Lakes Family Health (UGL) supported through funding from the Health Resources and Services Administration (HRSA). With parental/guardian consent, our Registered Nurse (RN) and Licensed Behavioral Health Practitioner can provide care conveniently within the school.
This allows for ease of access, fewer disruptions of regular routines and less loss of class time for students.

WELLNESS SERVICES PROVIDED*
- Screening/nursing assessments
- First aid for minor injuries
- Dispensing over the counter (OTC) medications under medical director standing orders
- Immunization assessment (review of record)
- Chronic care intervention
- Blood pressure monitoring
- Case finding and case management
- Point of care lab testing (upon request) and/or referral to other needed primary care and specialty medical services.
- Behavioral health services upon referral
* Primary Care Services are available in person and via telehealth visits upon request.

A provider is on-call after hours, on weekends and during holidays. For urgent after hours needs for family practice call (906)372-3025. For after hours needs for OB/GYN or Pediatrics please call (906)483-1000.
