Your First Visit
As a new patient, please arrive at least 15 minutes early to allow time to complete a healthcare questionnaire and new patient information.
At your first visit, we will ask you some brief screening questions to see if you qualify for programs that offer discounted rates for health care services.
Your first visit will offer you and your provider an opportunity to not only address your immediate health concerns, but to also discuss your health history and other measures you can take to get and stay healthy.
Bring these things to your first visit
- ID and insurance card (if you have insurance) and a list of your medications, supplements, and vitamins.
- Co-payment. You are responsible for co-pays, deductibles or full payment if we are not billing your insurance for the visit.
- Any questions for your provider.
- UGL has an affordable care program to assist our patients with the cost of healthcare. We provide assistance with enrolling in affordable health insurance, discounted prescription programs, and discounted rates for service.
