Upper Great Lakes Family Health is proud to share that two members of our team were recently recognized by the Michigan Primary Care Association (MPCA) for their extraordinary leadership and dedication to service. Trevor Hodges, CEO, received a Healthcare Leadership Award, and Sarah Tregembo, Practice Manager of UGL’s Gwinn and Sawyer Clinics, was honored with a Distinguished Service Award.
Representatives from the MPCA recently visited UGL’s Gwinn Clinic to present the awards.
Trevor and Sarah both exemplify the mission and values of UGL. Their leadership, service, and dedication reflect the meaningful impact healthcare professionals can have within their organizations and across the communities they serve.
Please join us in congratulating Trevor and Sarah on these well-deserved honors.
Trevor Hodges Receives Healthcare Leadership Award
Trevor Hodges exemplifies servant leadership through a hands-on, collaborative approach rooted in listening and engagement. He regularly connects with staff across all levels of the organization to better understand day-to-day challenges, gather feedback, and ensure frontline perspectives help shape organizational improvements.
Beyond internal operations, Trevor’s leadership extends throughout the broader healthcare and community landscape. He works closely with healthcare partners and community leaders with a continued focus on improving care and strengthening systems for the people they serve.
His commitment is visible not only in strategy and advocacy, but also in action — whether supporting organizational initiatives or stepping in during unexpected emergencies. From attending community events to personally repairing plumbing late into the night after an office flood, Trevor consistently leads with empathy, integrity, and resolve.
The Healthcare Leadership Award recognizes leaders who inspire trust, foster collaboration, and lead with purpose — qualities Trevor continues to demonstrate throughout his work and service.
Sarah Tregembo Honored with Distinguished Service Award
Compassionate, hardworking, inspirational, and collaborative — these words describe Sarah Tregembo, Practice Manager of UGL’s Gwinn and Sawyer Clinics. Born and raised in Gwinn, Michigan, Sarah’s journey came full circle when she transitioned from being a patient at the Gwinn Clinic to becoming a leader within the organization.
Since earning her nursing license in 2008, Sarah has steadily grown in her career, joining the Sawyer Clinic in 2016 as a Quality Coordinator before stepping into leadership roles within the organization. In addition to her administrative responsibilities, Sarah regularly steps in to support clinical operations, helping ensure patients continue to receive uninterrupted, high-quality care while supporting her teams during challenging times.
Sarah consistently puts the needs of her team and community first and approaches challenges with compassion and determination. Her peers often describe her as a source of strength and encouragement.
Her dedication extends beyond the clinic walls. Sarah helps lead community initiatives like the Sawyer Community Garden, supports internal events such as the employee picnic, and has served on the Gwinn School Board and the District 11 Little League Board — all while raising a family and remaining deeply involved in her community.
The Distinguished Service Award honors individuals whose commitment makes a lasting difference, and Sarah’s impact is felt every day by her team, patients, and community.
